This is a fan-made remake that you can find as a secret bonus in the ambitious Mjolnir mod for Quake. If you shoot a hidden button—which isn’t hard to find—in the mod’s episode hub, you’ll gain access to a secret room where you can play remakes of the first two episodes of Heretic, powered by the Quake engine. In this gameplay video, I play through the entire “City of the Damned” episode with no prior experience, using the Ironwail source port at 4K resolution.
